Chris Ndakwe brings over 25 years of experience in mental health and behavioral rehabilitation to his role as a Senior Transition Specialist. With Master’s degrees in Sociology and Public Health, and as a graduate of Mom’s House, Chris is a passionate, ethical advocate. He focuses on reducing hospital readmissions through safe discharges and transitional placements, guiding families with the absolute dignity and professional expertise they deserve.

How quickly can you help if we have an urgent hospital discharge?
We specialize in urgent placements. For hospital discharges or skilled nursing transitions, we typically respond within 2 hours and can often secure a safe placement within 24–72 hours.
Do you only work with private pay families?
While many assisted living communities are private pay, we also help families explore VA Aid & Attendance benefits and Medicaid Waiver options. We connect you with the right financial resources and professionals.
Are you affiliated with any specific community?
No. We are 100% independent. We work with over 20 vetted communities so we can match your loved one to the right fit based on care needs, location, and budget—not steer you to a single corporate location.
What if my parents refuse to move?
